New York State Infertility Demonstration Project (New York State Grant Program)

The NYU Langone Fertility Center is currently accepting applications for the New York State Infertility Grant Program for the contract year ending 9/30/2014. Applications will be reviewed and qualified patients will be accepted until either the allocated funding is exhausted or the grant expires. Currently the 2013 income tax returns, both NYS and Federal 1040 will be required for income verification and determination of the patient cost share. If your income status has changed and differs from your tax returns for both patient and partner in 2013, you will be required to provide alternate proof of income.

You must be 21-44 years of age, a New York State resident, have a 2013 total household income of less than $200,000, and be covered by a commercial insurance carrier (not Medicare or Medicaid, i.e. HealthPlus). Your medical history should include four cycles of ovulation induction with or without insemination unless you have documented tubal disease or documented severe male factor infertility. If you have had three or more failed IVF cycles you are not eligible for the grant. Individuals with a high FSH or with a medical condition that contraindicates pregnancy are ineligible. When submitting the application, you must also submit your 2013 New York State Tax Return and your Federal 1040 return together with a legible copy of your insurance card (front and back). Residential address must be verifiable by NY State driver's license or available utility, rent or property tax bill.

Once the application is reviewed, you will be contacted about your eligibility, provided there is funding available. At that time, you will be required to meet with our billing staff and provide the documentation necessary to determine your financial eligibility and sign an attestation form. The financial portion includes three segments: insurance reimbursement, patient co-pay and the New York State payment. The patient co-pay percentage is pre-determined by the State, based on annual income for both prospective parents. You will be apprised of this amount, and will be expected to provide a credit card for charging at the time of the initial consult. Insurance claims for services rendered will be submitted to your insurance carrier; any insurance EOBs or reimbursement must be endorsed to NYU Langone Fertility Center. Insurance reimbursement levels will reduce the amount of your co-pay.

This program covers most services associated with IVF for the usual patient, including drugs, anesthesia, ICSI, ultrasound scans and hormone assays, egg retrieval, embryo transfer and one year of embryo storage; however, there are exclusions- i.e. donor egg fees, egg freezing, sperm storage, ovulation induction, insemination, general gynecological care, pre-implantation genetic diagnosis, surgery, MESA, PESA.

The patient grant portion of this project is funded by the New York State Department of Health.
